      Overview

In its mission to support the independence and improve the life quality of people with disabilities, Community Residences serves over 1,100 consumers annually in a variety of programs from Baltimore to Richmond.

We welcome the opportunity to work with you and your loved one(s) in order to find the right match between an individual’s needs and available services, whether those services are provided by Community Residences or whether we are able to refer you to other qualified providers.

      MENTAL HEALTH SERVICES

      Residential Programs:
Community Residences operates several mental health residential programs in Arlington County, Virginia that are open only to residents of Arlington County. The programs span a continuum of housing options from the least restrictive residential settings that are transitional in nature to supportive assisted living facilities for individuals with mental health diagnoses who require more intensive assistance. Our philosophy is to provide a normalizing, home-like environment where we assist consumers in learning skills that promote community tenure and integration, better health management and opportunities to fully engage in their recovery process. Varying levels of care are provided depending upon the intensity of need and include supportive services, coordination of medical needs, therapeutic services, nursing care and assistance with daily structure. Referrals to any of CR's mental health residential programs must be made by the consumer's Arlington County Behavioral Healthcare Case Manager.

      Community Support Services:
Community Residences provides recovery-oriented community support services to individuals with serious mental illness who need support to live as independently as possible in their community. These services focus on approaches that enable an individual to experience their full potential through individual recovery. Support staff assist with daily and social skill development, health management, financial management, housing assistance and personal goal-setting.

Case Management Services: Community Residences offers case management services to adults who have been diagnosed with serious and persistent mental health disorders. CR provides Case Management that builds upon each individual's strengths, and that is comprehensive, flexible and mobile. Assistance is provided with accessing private, local, state, and federal resources and entitlements.

Referrals: Please direct any inquiries about services for mental health consumers to CR's Clinical Director of Mental Health Programs, Alice Straker, at 703.841.0607, ext. 11 or astraker@comres.org.

SERVICES FOR INDIVIDUALS WITH INTELLECTUAL DISABILITIES

Intermediate Care Facilities (ICFs): These programs provide long-term residential services to consumers who have Intellectual Disabilities and co-occurring medical and/or behavioral needs. Consumers are supported in developing a person-centered plan of care to address their needs and desires in areas such as self care, socialization, financial management, communication and mobility. ICF staff provides ongoing active treatment based on consumers’ Individual Service Plans and offers a range of activities to encourage community integration and independence. The ICF team includes the Program Manager, Program Nurse, Social Worker, Recreation Director, Clinical Director, Counselors and Aides. Consumers must be eligible for Medicaid to receive funding for this level of service.

Group Homes:  These programs provide residential services to consumers who have Intellectual Disabilities and who need assistance and training to maintain and learn skills related to activities of daily living and use of community resources. Consumers have Support Coordinators assigned by the Community Services Board who are active in their treatment and who support the consumer in developing a person centered plan of care that best meets his or her individualized needs. The Program Manager, Counselors, Aides, Nurse and other people important to the consumers are all active members of the consumers’ team. Each group home is unique and serves consumers with a wide range of skills and needs. Funding for consumers in these programs comes through Medicaid Waiver, county contracts or through private pay.

Community Support Program: This program provides in-home support services to consumers who have Intellectual Disabilities and who live in independent settings. The program currently only supports consumers in Arlington County. Consumers have Support Coordinators assigned by the Community Services Board who support them in the development of a person centered plan of care. Services provided can range from several hours per week to several hours per day. The goal of the service is to provide support to consumers with activities of daily living and use of community resources. Funding for consumers in this program comes through Medicaid Waiver, county contracts or through private pay.

Referrals:  Please direct any inquiries related to referrals to the above programs serving individuals with Intellectual Disabilities to CR's Director of Admissions and Social Work, Lorna Ballard, LCSW at 571.236.4389 or lballard@comres.org.
